The AmeriCorps Whole Health Management Project will focus on the Corporation for National and Community Service (CNCS) focus area of Healthy Futures. Within this project, AmeriCorps members will work directly with Valeo consumers, who are part of vulnerable populations, providing health and wellness supports to better address the comprehensive health needs of those with mental health and substance use issues. Activities/interventions the AmeriCorps members will provide consist of: Basic health assessments and monitoring (e.g., blood pressure, blood glucose, height, weight, BMI, etc.); Health education, support and encouragement; Assistance in accessing and following through with primary care appointments; Resource acquisition; and, Collaboration between primary and behavioral health care. The AmeriCorps Whole Health Management Project will engage five AmeriCorps members to work between September 1, 2014 and August 31, 2015 at Valeo Behavioral Health Care. AmeriCorps members will actively work with consumers to address the major preventable risks for people living with mental illness: smoking, obesity, diabetes, elevated cholesterol, and high blood pressure. AmeriCorps members will also work to address barriers such as poverty, unemployment, underemployment, lack of transportation, lack of knowledge of the health care system, etc. that make access to health care difficult to obtain. AmeriCorps members will be expected to share their AmeriCorps experience within the agency as well as the community. Extensive training will be provided.
